DESCRIPTION
Hospitals for a Healthy Environment (H2E) recognized a number of factors in developing the
Hazardous Chemical Waste Prioritization Tool. Regulatory burden, employee health and safety, the
long-lived toxicity of PBT chemicals and volume are the factors used. These factors are further
defined below.

Regulatory burden1: Hazardous chemicals are subject to a variety of regulations when
discarded. The chemicals that are regulated under EPA's Resource Conservation and
Recovery Act (RCRA) are one set of criteria used. RCRA "P" listed chemicals, due to their
acute toxicity and increased regulatory burden were considered in an additional category.

If the chemical is considered an acutely toxic pesticide, Category I by the US EPA Office
of Pesticide Programs, it then is also a hazardous waste when disposed of. See

One point was given if the chemical is a hazardous waste. An additional point was given if
the waste is "P" listed. RCRA listed hazardous wastes and characteristics can be found in
Appendix A. More detailed information can be found on EPA's web site at

Employee Health and Safety2: the Occupational Safety and Health Administration's
(OSHA) hazardous substance lists defined this criterion. The lists used were the OSHA
Pharmaceuticals List (OSHA Instruction CPL 2-2.20B CH-4) (Appendix D) and the Air
Contaminants list of 29 CFR 1910.1000 subpart Z (Appendix C).

Note: the presence of a chemical on a material safety data sheet (MSDS) indicates it is a
"hazardous substance" per OSHA criteria; hence presence on the "Air Contaminants" list
presents another hazard. One point was given if the waste is listed on OSHA's hazardous
substance list.

Persistent, Bioaccumlative and/or Toxic Chemicals (PBTs)3: Discussed on page 10 of
the Chemical Minimization Tool, PBTs were identified using the draft EPA-derived list of 53
chemicals targeted for elimination under the National Waste Minimization Plan. The full list
of PBTs can be found in Appendix E. One point was given if the waste is a PBT.

Volume4: Chemicals may present more of a concern by the nature of a larger volume being
used and ultimately disposed of. The H2E tool attempts to identify those large volume
chemicals. H2E acknowledges that volumes of chemicals used will vary with institution size.
However, the amounts of chemicals used within an individual institution will be
proportionate. For example, most institutions will use relatively large volumes of floor
stripper if they have hard floors, but use considerably lower volumes of furniture polish.

For the purpose of gauging, volumes have been broken down as follows: Large > 100
gal/year, Medium 50-100 gal/year; Small 15-50 gal/year; Very Small <15 gal/year. 1 point
for Large or Med. Volume, 0.5 point for small, 0 points for Very Small. Exception: for
pharmaceuticals where "large volume" was based on frequency of use.

Total: Each factor present for a given chemical was given one point (volume points given on
basis listed above). If the chemical scored 3 or greater it was identified as a "priority
chemical".


OTHER CRITERIA
Pesticides: Include antimicrobials and disinfectants. This class of chemicals is of special concern as
they are designed to kill. Some pesticides such as Lindane are included as hazardous chemicals under
RCRA, many others though, are not. However, if a pesticide is labeled "acutely toxic" it must be
managed as a hazardous waste.5 Pesticides are used throughout healthcare, especially in the form of
disinfectants and sterilants.

Pesticide use should be thoroughly assessed within each department. The proper pesticide and its
application should be reviewed. Employees must be trained on proper use. Knowledge of pesticides
and their relative toxicity can be assessed using "Leachability" criteria and label review. Choose
pesticides with the lowest leachability factor possible that meets your need. Pesticides with lower
leachability stay in the soil allowing them to be effective and reducing the amount that contaminates
ground or surface water. Implement integrated pest management practices.

Aquatic Toxicity: In some cases, chemicals generated at a healthcare facility are poured down the
sewer. Most chemicals are not adequately treated at wastewater treatment plants and hence
discharged to surface waters such as lakes, rivers and oceans. Many of these sewered chemicals may
become regulated as the Clean Water Act provisions are effected. The Aquatic Toxicity chemical list
is an EPA-derived list found at www.epa.gov/ecotox. The database allows you to retrieve
information by CAS number.

LIMITATIONS OF THE PRIORITIZATION TOOL
The number of chemicals in use today has been estimated at 80,000! The number of chemicals used
in healthcare ranks in the thousands as well. H2E recognizes that not all hazardous aspects of
chemicals are identified using this tool. Also not all chemicals used in healthcare have had the tool
applied to them

In part, due to the surfeit of chemicals, not all are regulated. New chemicals enter the market and
workplace much more rapidly than regulatory agencies can test and assess them. Hence, although a
chemical may not be technically regulated, such as some of the newer pesticides or disinfectants by
OSHA or RCRA, it is important that its hazards be recognized. Review of a product's material safety
data sheet (MSDS) can help identify the steps necessary to ensure both occupational and
environmental safety.

Carcinogens are another category of chemicals not generally identified using the H2E Chemical
Minimization Prioritization Tool. The tool is designed to identify those chemicals that present an
immediate or acute hazard. Carcinogens frequently present a hazard associated with chronic use;
hence they are not usually covered by the regulations used for this tool. Additionally, many
carcinogens have been identified only recently and the regulatory agencies have not yet had the
opportunity to include them in the rules.

The tool has been applied using lists of chemicals from a variety of healthcare institutions. H2E
believes the chemical lists are representative, that is, most healthcare institutions will be engaged in
similar processes. However, the exact chemicals or combination of products will vary from institution
to institution. Hence, the prioritized chemical lists are designed as guidance to assist healthcare
organizations with identifying those chemicals that present significant concerns.

H2E identified a number of chemicals used in healthcare. Each chemical was then cross-referenced
against the criteria listed above. For each match, the chemical was given a point. Those chemicals
with a score of 3 or higher have been targeted as "Priority" Chemicals to be reduced or eliminated.

An example is given below.

Results:
Generally those chemicals that are PBTs or are found on the RCRA P-list were those that scored
highest using the H2E Tool. The chemicals that scored 3 or more, the departments where they may
be found, and their typical use are listed in Table 3 of the Chemical Minimization Chapter.
Minimization tips for priority chemicals may be found in the individual department areas of the main
chapter. Priority Chemicals may be found in other departments, but generally not in significant
volumes.
